FAST TRAC
System of Care Grant
What services it provides and what the future might bring

Clermont County Crisis Hotline and Mobile Crisis Team
This program focuses on the FAST TRAC System of Care Grant and will give an overview of services provided or enhanced by the grant. It will address how FAST TRAC is working toward sustainability of the Family Support component. The discussion will also cover Families Connected as a resource for families, and future plans laid out for the organization. Families Connected has supported families through a combination of volunteering and contracts since it was established.
About the presenter:
Jean Houston, Co-Founder of Families Connected of Clermont County, Inc. Established in 1999 to support families who have a member with a disability.
Jean has spent many years in support of families through training, resources, peer support, and has been actively involved within Clemont County since 1978. Her initial efforts resulted in a small grant from the MHRB for a conference to support families, and eventually received an inkind grant by the Clermont County Board of Developmental Disabilites to house their organization. They continue to provide services from the Wildey Center and in addition to the Help Me Grow Program, offer services to support families in educational systems. In 2010, Jean was contracted to fill the position of the Lead Family Contact for the SAMHSA Grant (Substance Abuse and Mental Health Services Administration) received by Clermont County by the Clermont County MHRB to support families who have children with mental health and substance abuse challenges. Jean supervises 4 (four) Peer Support Partners supporting families enrolled in the Wraparound Program through the grant.
